The List of Required Information For Registration Completion Appointments

  1. A readable copy of your DUI Arrest Report / Probable Cause Report. This can be obtained from the agency that arrested you or from your attorney. A crash report alone is not sufficient unless you were taken from the scene in an ambulance and received a DUI ticket later.
  2. A Lifetime Driving History, which cannot be more than 30 days old when you come to complete registration. This can be obtained from any Florida Clerk of Court office or our DUI office for $15.00 and must be obtained even if you have never had a Florida driver’s license. If you have held a license in any other state(s) you must obtain a certified driving history from them.
  3. A copy of your Court Order if you have completed the court process.
  4. Your current Picture ID.
  5. Your printed receipt from your online pre-registration.
  6. We cannot make copies of any of the above information for you.
  7. We cannot complete your registration process if any of the above items are missing. Extra time will be allowed for you to obtain #1, if you have registered online to meet the 10 day requirement for the Administrative Suspension Waiver Law (effective 07/01/13).

Ten Day Eligibility Review For Restricted Driver License/Waiver of Administrative Suspension

Amended Florida Statutes effective July 01, 2013
322.2615(1)(b)(3) – Suspension of license; right to review. –
In addition to a driver requesting a formal or informal review of the administrative suspension by the department (DHSMV) within 10 days of issuance of the notice of suspension, driver may request a review of eligibility for a restricted driving privilege.

322.271(7)(c) – Authority to modify revocation, cancellation, or suspension order.–
Acceptance of the reinstated restricted driving privilege (business purposes only) is deemed a waiver of the right to formal and informal review under F. S. 322.2615

As of July 01, 2013, a person may request a review of eligibility (for a first offense DUI, only) to have the ‘hard time’ portion of their suspension set aside. That means they won’t have to go without a driver’s license for the traditional 30 or 90 days (based on a BAC reading or a Refusal).

  • This process must be completed within 10 days of the person’s DUI arrest.
  • The person must request the review in person at the Bureau of Administrative Reviews (BAR) in Tampa*
  • The person must show the BAR proof of enrollment in a local DUI program or a copy of their online receipt.
  • The person must take the BAR approval to DHSMV to get their hardship permit.
  • The person must come to a local DUI office in Polk, Hardee or Highlands to complete the registration process, within 5 days of the online registration.

Welcome to our online DUI pre-registration process

Tri-County Human Services, Inc., (TCHS) DUI Program is licensed and monitored by the Florida Department of Highway Safety and Motor Vehicles (DHSMV), according to Administrative Code 15A-10, pursuant to Florida Statutes.

The DUI Program consists of:

  • A 12 hour Level 1 or a 21 hour Level 2 class and a 1 hour substance abuse evaluation.
  • Level 1: 1st time offenders with no prior arrest requiring a DUI program or no prior DUI program attendance.
  • Level 2: Repeat offenders with more than 1 offense requiring a DUI program or prior DUI program attendance.
  • Call the Lakeland DUI office Mon.-Fri., 8:00-5:00 @ 863-701-1919(0) for questions re: your program level.

The DUI Payment Process consists of:

  • Credit/Debit Card payment ONLY-Visa/MasterCard/Discover .
  • Print Receipt as your proof of registration.
  • Level 1 fee - $340.00, Level 2 fee - $504.50. Fees are due in full on the final registration page.
  • Fees above include a $10.50 or $15.00 service fee respectively and a $7.00 online processing fee.
  • All fees paid to Tri-County Human Services, Inc. DUI Program are non-refundable.
  • Payment of fees is acknowledgement and agreement to the DUI Program Rules and Regulations.

The DUI Registration Process consists of:

Pre-Registration

  • To register for this DUI program you must live, work, or attend school in Polk, Hardee or Highlands counties.
  • Submitting data online and making your online payment only begins the registration process.

Registration completion

  • Call our Lakeland DUI office @ 863-701-1919(0), Mon. - Fri, 8:00-5:00 to schedule a registration appointment and to obtain the list of required items you will need to bring to your first appointment.
  • Your registration completion takes up to 1 ½ hours and includes a breath alcohol test.
  • You must contact this office within five (5) days of your on-line payment/pre-registration.
  • Failure to complete the DUI program within 90 days of this online payment will result in forfeiture of all fees, violation of probation and license cancellation by DHSMV.
  • Please Note - The BAR will require additional registration information, including proof of class assignment, for the 10-day Eligibility Review For Restricted DL/Waiver of Administrative Suspension laws: For more information, contact our office at the number above.
  • Payment in full is due on the final page of this registration. Payment of fees is acknowledgement and agreement to the program rules and regulations. All fees payable to Tri-County Human Services, Inc are non-refundable, even if you are found not guilty of the DUI and your administrative suspension is invalidated.
